Local Care Partnership
Central North Primary Care Network is one partner in the Central Local Care Partnership (LCP). A LCP is the term used in Leeds to describe our model of joined-up working to deliver local care for local people; working in and with local communities.
Central covers the areas of North Leeds, Meanwood, Oakwood, Scott Hall and Little London and all in-between. As well as the Primary Care Network, partners include community health services, adult social care, local councillors, pharmacy, third sector organisations and community groups, who want to work together to improve the wellbeing of our local population.
